Enrollment fees are non-refundable and billed once your onboarding phone call is complete with our staff.

Monthly fees are billed at your new patient appointment or 2nd month of enrollment whichever comes first.

Important Information For New Patients:

1. We welcome all patients irrespective of insurance status. As a concierge style membership based practice, we do not bill insurance and we do not charge co-pay or deductibles for any of your office appointments. All your office appointments are covered under your monthly fee.

2. Monthly membership fee will be billed on the 1st of the month. Patients can make payment either with credit card on file or use ACH withdrawal which we keep on file as well. We require a method of payment during your enrollment. We do not make any charges until you have completed your onboarding phone call with our staff.

3. Discounted Services:

a. All members have access to discounted pricing for lab tests, in-office pharmacy medications, and negotiated prices for imaging studies. We maintain full transparency with our pricing, ensuring members are informed of any incidental costs at the time of service. All incidental charges, including labs and medications, are billed at the time of service.

b. For our patients with insurance (Private Insurance, Medicare or Medicaid), you can use your insurance for labs, imaging studies and medications per your preference.

4. Billing for New Patients:

a. We will contact you shortly after enrollment to complete your onboarding phone call. Once onboarding call is complete, we will bill your one-time non-refundable enrollment fee to reserve your membership spot.

b. Your first month membership fee is billed at your new patient appointment or your 2nd month of enrollment whichever comes first. If you are joining in the middle of the month, that first month payment is prorated.

c. Discounts:

1. If you pay for 11 months up front, you get your 12th month free

2. $10 OFF monthly for spouse/partner

3. If >3 children enrolled, cost is $70/month for 3rd child
